Please submit the following documents with three copies of the thesis: the duly completed Thesis Submission Form for Examination with recommendation from supervisor/s, relevant pages of Student Supervisory Record Book on details of supervision, Plagiarism Check report of your thesis and any other documents granting approval for the use of information and materials published earlier. If a student submit a thesis for re-examination after major corrections, he/she should use the Thesis Submission Form for Re-examination.
